1. What is Pascorbin and what is it used for?

Ascorbic acid (vitamin C) is a water-soluble vitamin.

Pascorbin is used:

  • For the treatment of clinical conditions of vitamin C deficiency that cannot be corrected by diet or oral supplements.
  • In cases of methemoglobinemia in childhood.

2. What should you know before using Pascorbin?

The following cases describe situations in which Pascorbin can only be used under certain conditions and with special caution. Please consult your doctor about this! This also applies if you have had these conditions in the past.

Pascorbin can only be used after consulting your doctor in the case of oxalate kidney stones and certain diseases in which the body stores too much iron (thalassemia, hemochromatosis, sideroblastic anemia).

Warnings and precautions

Special note for diabetics: After parenteral administration of ascorbic acid, the blood glucose detection reaction is impaired.

Intravenous injection of large doses of P-ascorbin can lead to acute renal failure due to kidney stones, by precipitation of calcium oxalate crystals in the kidneys. This has been observed even at doses of 1.5-2.5 g. Patients with pre-existing renal insufficiency (renal failure) are particularly at risk.

An adequate fluid intake (approximately 1.5 to 2 liters per day) should be ensured.

In patients with recurrent kidney stone formation, the daily dose of vitamin C should not exceed 100 to 200 mg.

Patients with severe or end-stage renal failure (dialysis patients) should not receive more than 50 to 100 mg daily.

In patients with erythrocyte glucose-6-phosphate dehydrogenase deficiency (an inherited disorder of red blood cell metabolism), severe hemolysis (destruction of red blood cells) has been observed in isolated cases with high doses of vitamin C (more than 4 g daily). Therefore, exceeding the daily dose of 100–500 mg of ascorbic acid should be avoided.

In patients with known respiratory problems (especially due to impaired respiratory function, such as in asthma, chronic bronchitis or smoker’s cough), in isolated cases, acute respiratory distress may occur during treatment with high doses (from 7.5 g) of Pascorbin. Therefore, it is recommended to start treatment with lower doses in these patients.

Pascorbin should not be mixed with drugs whose effect is reduced by chemical reduction.

Information for children
  • In children under 12 years of age, the intravenous dose of ascorbic acid should not exceed 5-7 mg/kg body weight per day when administering parenteral nutrition.
  • In the treatment of methemoglobinemia in children, the daily dose should not exceed 100 mg/kg body weight.
Information for the elderly
  • Sufficient fluid intake (approximately 1.5 to 2 liters per day) must be ensured.

Note:

  • After administration of doses of one gram, the concentration of ascorbic acid in the urine can increase so much that the measurement of various clinical-chemical parameters (glucose, uric acid, creatinine, inorganic phosphate) is disrupted and distorted.
Using Pascorbin with other medicines
  • The effects of the following groups of drugs or preparations may be influenced by simultaneous treatment with Pascorbin.
  • Pascorbin may chemically modify other drugs (due to its high redox potential); therefore, compatibility with other drugs must be checked in each individual case when administered concomitantly.
  • Ascorbic acid may affect the action of anticoagulants.
  • Concomitant administration of ascorbic acid and fluphenazine leads to a reduced plasma concentration of fluphenazine; concomitant use of ascorbic acid and acetylsalicylic acid leads to a reduced excretion of acetylsalicylic acid and an increased excretion of ascorbic acid in the urine.
  • Salicylates inhibit the absorption of ascorbic acid in leukocytes (white blood cells) and platelets (platelets).

During chemotherapy, higher doses of vitamin C should be administered at different times (1-3 days, depending on the half-life of the chemotherapy drug), as clinical data on possible interactions are lacking.

Please inform your doctor or pharmacist if you are taking/using or have recently taken/used any other medicines, including medicines obtained without a prescription.

Using Pascorbin with food and drink
  • No special precautions are required.
Pregnancy and breastfeeding
  • Pregnancy: During pregnancy, the daily intake of ascorbic acid should not exceed 100 to 500 mg.
  • Breastfeeding: During breastfeeding, the daily intake of ascorbic acid should not exceed 100 to 500 mg.
Ability to drive and operate machinery
  • No special precautions are required.
Pascorbin contains sodium

One ampoule with 5 ml solution for injection contains 4.2 mmol (97.2 mg) sodium and one vial with 50 ml solution for injection contains 42.3 mmol (972 mg) sodium.

This should be taken into consideration by patients who need to follow a low-sodium diet.

3. How should Pascorbin be used?

Always use Pascorbin exactly as described in this leaflet.

Please ask your doctor or pharmacist if you are not sure.

Pascorbin is usually administered by a doctor.

Unless your doctor has prescribed otherwise, the usual dose of Pascorbin is for peripheral venous injection.

Unless otherwise prescribed, slowly inject 5 ml intravenously daily, up to 50 ml of injection solution as an additive to an infusion.

To prevent vitamin C deficiency during prolonged artificial (parenteral) nutrition, adults receive 100 to 500 mg of ascorbic acid intravenously per day (corresponding to 0.7 ml to 3.3 ml of Pascorbin).

Children receive 5 to 7 mg of ascorbic acid/kg body weight intravenously per day.

Methemoglobinemia in children: administer a single intravenous injection of 500 to 1000 mg of ascorbic acid (corresponding to 3.3 ml to 6.6 ml of P-ascorbin).

If necessary, the administration should be repeated with the same dose.

A dose of 100 mg/kg body weight per day should not be exceeded.

Method of administration

Pascorbin is injected slowly intravenously (into the veins) by your doctor or other healthcare professional.

Duration of use

Your doctor will decide the duration of use depending on the progression of the disease and hematological values.

If you use more Pascorbin than you should…
  • The use of Pascorbin should be discontinued!
  • Generally, no further measures are necessary, except in the case of hemolysis or kidney stones (see section “Warnings and precautions”).
  • If you experience pain in the kidney area or difficulty urinating after a high dose of Pascorbin, you should contact a doctor immediately!

4. What side effects are possible?

Like all medicines, Pascorbin can cause side effects, although not everybody gets them.

The following categories are used to describe the frequency of side effects:

  • Very common: more than 1 in 10 patients treated
  • Common: 1 to 10 in 100 patients treated
  • Uncommon: 1 to 10 in 1,000 patients treated
  • Rare: 1 to 10 in 10,000 patients treated
  • Very rare: less than 1 in 10,000 patients treated
  • Not known: frequency cannot be estimated from the available data.
Possible side effects
  • Very rarely, hypersensitivity reactions (e.g., difficulty breathing, allergic skin reactions) have been observed.
  • Very rarely, high doses of ascorbic acid may cause gastrointestinal disturbances (e.g., nausea, vomiting, diarrhea).
  • In isolated cases, short-term circulatory disturbances may occur (e.g., dizziness, nausea, visual disturbances).
  • In very rare cases, reactions such as chills and fever have been observed in acute infections.
Countermeasures
  • Stop the injection or infusion immediately.

5. How should Pascorbin be stored?

Keep out of the sight and reach of children!

Do not use this medicine after the expiry date which is stated on the carton and container after “EXP”.

The expiry date refers to the last day of that month.

Storage conditions

Do not store ampoules or injection vials at temperatures above 25 °C.

Protect ampoules or injection vials from light.

Instructions for use after opening or preparation
  • The preservative-free solution for injection is for single use only.
  • It should be used immediately after opening the ampoule or injection vial.
  • Any unused portion should be discarded.
  • Never throw away medicines via wastewater (e.g. not down the toilet or sink).

Ask your pharmacist how to dispose of your medicine when you no longer need it. By doing so, you help protect the environment.

6. Package contents and additional information

What does Pascorbin contain?
  • The active substance is ascorbic acid. 1 ml of solution for injection contains 150 mg of ascorbic acid.
  • The other ingredients are: sodium bicarbonate, water for injections.
What does Pascorbin look like and the contents of the package?
  • Brown ampoules or a brown glass bottle containing a clear, yellowish liquid.

Pascorbin is available in boxes of 5 ampoules/5 ml, 10 ampoules/5 ml, in hospital boxes of 100 ampoules/5 ml, 1000 ampoules/5 ml, as well as in 50 ml vials and in multipacks of 20 and 60 50 ml vials.

Not all pack sizes may be marketed.
